To request the re-order of collection pack items please complete the form below with item quantities and dispatch details.

We are no longer able to provide pre-paid special delivery boxes due to changes at Royal Mail. 

Please note:

All full collections packs will  contain Streck tubes for collection of peripheral blood.

Streck collection packs only contain Streck tubes and the correct borcodes. They will also be used for peripheral blood for patients with solid tumours who are not on clinical trials.

PLEASE DO NOT USE STRECK TUBES FOR ANY PATIENT ON FaR-RMS, HRNBL2 or BEACON.
