To request the re-order of collection pack items please complete the form below with item quantities and dispatch details.

Please note:

All full collections packs will now contain Streck tubes instead of EDTA tubes for collection of peripheral blood.

Streck collection packs only contain Streck tubes and the correct borcodes and can be used to top-up older packs. They will also be used for peripheral blood for patients with solid tumours and we will notify you when this is running.
